WA Paediatric Inpatient Medication Chart
Charts and Guidelines
The following paediatric medication Charts have been developed and mandated for use in WA public hospitals:
- WA Paediatric Short-Stay Medication Chart (PDF 273KB)
- WA Paediatric Long-Stay Medication Chart (PDF 274KB)
In December 2008 the Australian Health Ministers agreed to use a common paediatric medication chart in all public hospitals where paediatric pateints are treated. A new standardised Paediatric Long-stay and Short-stay Medication Chart will be rolled out across WA Health beginning in July 2009. The charts have been created via consultation with stakeholders including clinicians, pharmacists and administrators from WA Health
Private hospitals in WA may adopt the WA Inpatient Medication Chart and WA Paediatric Long-Stay Medication Chart. However, the charts will need to be modified by private hospitals to facilitate the submission of prescriptions for inpatient medication to the Health Insurance Commission, as applicable.
Guidelines
The OSQH has developed Guidelines to assist WA hospitals and WA clinicians to implement and use the WA Inpatient Medication Chart and WA Paediatric Long-Stay Medication Chart at the local level.
