WA Long Stay Chart
After the successful implementation of the National Impatient Medication Chart (NIMC), a number of Health Services identified the need for a standardised Long Stay Chart. This chart is designed for patients for whom a hospital stay of longer than 10 days is expected, such as patients in rehabilitation and geriatric units; and residential type patients in aged care facilities.
The WA Long Stay Chart (WA LSC) has been developed based on the safety principles of the NIMC.
WA Variable Dose Chart
The variable dose medication chart was developed in consultation with the NIMC Working Group and a number of stakeholders and is to be used in conjunction with the WA Long Stay Chart and/or National Inpatient Medication Chart. The use of the WA Variable Dose Chart will not initially be mandated, however its use is encouraged in all appropriate settings.
