WA anticoagulation medication chart and information booklet

Background

An initiative by an expert working group of the Western Australian Medication Safety Group to improve the safety of anticoagulation medication use has led to the development of a standard Anticoagulation Medication Chart (AMC) for Western Australian public hospitals. The AMC complements, and should be used in conjunction with the National Inpatient Medication Chart (NIMC).

The AMC enables the documentation of all anticoagulant medications in one document, i.e. the prescription of warfarin, IV heparinoids and subcutaneous heparinoids (including LMWH).

The chart includes a number of evidence-based design features to improve the safety and quality of anticoagulant drug use, and ultimately reduce the risk of patient harm. These features include:

  • the safe and effective prescribing of all anticoagulant drugs documented on one form
  • incorporation of guidance and decision-support on the use of anticoagulant drugs
  • supply of evidence-based guidelines at the point of prescribing
  • allows standardised, legible, clear and accountable prescription orders - including an ability to provide for multiple dose alteration in a safe manner
  • improves the recording of dose administration

The chart also includes information and prompts for providing patient education, and for providing relevant information to the patient's general practitioner to ensure continuity of care.
