Generic consent form
- Form A – Patient consent to treatment or investigation (PDF 45KB)

- Form D - Authorisation to proceed with surgery on a patient without a valid consent form (PDF 43KB)

New procedure specific information sheets
- Provide information about specific procedures in simple language
- May be used by health professionals to provide information to patients when gaining their consent to treatment
- Have been purchased from EIDO Health Care under copyright and adapted for the Australian context
- Are accessible only to health care professionals working in WA public hospitals via the Department of Health intranet site only

Procedure specific consent forms
- Will be replaced by procedure specific information sheets (PSIS) and model generic consent forms
- Procedure specific consent forms developed previously will not be updated or supported after the release of PSIS
- Forms may be amended and used as information sheets if endorsed by Area Health Service / Hospital Executive
