Procedure specific consent forms

Area Health Services should note that the suite of procedure specific consent forms (PSCFs) developed by the Department of Health in 2002 will no longer be updated or supported after the release of the new Consent Policy (2006). These forms have been replaced by new procedure specific information sheets and model generic consent forms.

However, if there is a need, health professionals may choose to use or translate these PSCFs or others into information sheets. If they do so, it is their responsibility to ensure that any amendment to the forms are endorsed by Area Health Service or Hospital/Health Service Executive.
