Reporting requirements

All hospitals and health services are required to report Sentinel Events as specified to the Director, Office of Safety and Quality in Healthcare within seven (7) working days of the event occurring. Notification should be made via the Sentinel Event Notification Form and include the hospital identification code, the time and date on which the event occurred, A brief description of the event, whether the investigation will be conducted under qualified privilege and if it is a Coroner's case. The hospital identification code is the establishment code listed in the Hospital Morbidity Data System (HMDS) Reference Manual July 2002.

A Sentinel Event Report, including recommendation and action plan is to be forwarded to the Director, Office of Safety and Quality in Healthcare within 45 working days of initial notification of a Sentinel Event.
