Sentinel event reporting
A sentinel event is a rare event that leads to catastrophic patient outcomes.
Sentinel event reporting was implemented in Western Australia in 2003. This reporting mechanism targets the most serious adverse events and ensures priority is given to thorough investigations and identification and sharing of prevention strategies where they exist. The mandatory nature of this system supports tracking of trends over time and some evaluation of interventions.
