Clinical incident reporting tools

A range of complementary incident reporting systems have been implemented in Western Australia to capture complex incident information from a range of services and sources. The key systems for incident reporting are:

• Clinical Incident Management System (AIMS)
• WA Audit of Surgical Mortality (WAASM)
• Sentinel event reporting
• Severity Assessment Codes (SAC)
• Coronial Liaison Team
• Healthcare-associated Infections Surveillance of WA (HISWA).
  HISWA was implemented in 2005 and complements CIMS by providing specific details of nosocomial infections that help target training to reduce healthcare associated infections in WA public hospitals. Mandatory reporting of MRSA through HISWA permits tracking of MRSA infection rates over time.
